First of all one person is chose as the 'doctor'. The doctor then goes into another room or somewhere that he cannot see what is going on.
Meanwhile the group is standing in a circle holding hands; their goal is to twist themselves up so that it will be almost impossible for the doctor to 'fix'. To do so, one player could step over the hands of somebody else in the circle, another could go under, etc.. NEVER are they allowed to let go of each other's hands. When they have finished twisting themselves up as much as possible they will all shout out in unison 'doctor, doctor, we need help'. That is the 'doctors' cue to come out and try to put everyone back into their original positions.
